2) We ship you a pack of survey invitation cards (size of a standard business card) every quarter. Your staff gives a card to each patient right after the visit.
3) Patient visits our website, enters the unique code on the card and responds to the standard questionnaire in minutes.
4) We email your report SOLELY TO YOU (or to the appropriate person) every quarter; i.e. your results are CONFIDENTIAL. You can also access your results online anytime.
Typical response rates are 5% to 30%; so 10 patients/day = 30 to 180 responses/quarter.

We offer two monthly subscriptions plans (we do NOT charge patients):
$65/month per doctor includes 1,000 invitation cards per quarter (about 15 cards/day).
$90/month per doctor includes 2,000 invitation cards per quarter (about 30 cards/day).
We accept all major credit cards.
Note that cards are imprinted with the name of a specific doctor. If you want to use our services for more than one professional, you will need an individual subscription for each professional. If you want to subscribe multiple doctors, please contact us. We also offer quarterly and yearly plans.
