For security reasons, for now, having a valid code is required to take the survey. |

Why should I respond to the survey? |
- You help to improve healthcare and patient-doctor relationships. Your doctor cares about you and values your honest opinion.
- It is free, anonymous and it often takes less than five minutes to complete online.
- You may enter for a chance to win a gift card.
- Report For Doc, LLC is an environmentally and socially responsible company. We print our card in recycled paper and we support medical organizations. In 2010, donations are made to: (1) American Breast Cancer Foundation; (2) American Diabetes Association; (3) American Heart Association; (4) Leukemia and Lymphoma Society; and (5) Smile Train - cleft palate surgeries for children.

Are patients' survey responses anonymous? |
|
Yes. Patients’ specific responses are not shared with doctors. Doctors can only see the aggregate results of the survey and they can also read the comments that you provide. Hence, unless a patient writes her/his name in the text, the doctor will not know the author of the comment. If a patient decides to enter into the sweepstakes, the contact information is used only to contact her/him in case he/she wins a prize. Please see our Privacy Policy for more details. |

How do the sweepstakes work? |

When a patient responds to the survey, he/she may decide to enter into the sweepstakes to win a gift card. Drawings are made annually and winners will be notified. For detailed information, please see the Sweepstakes Rules. |
Can I make personal donations to the medical organizations supported by Report For Doc? |
Absolutely! If you want to make a donation to one of the medical organizations we support, we invite you to visit their website and donate directly – just click on the links on the bottom of this page. We sincerely appreciate your support. |
